Cover Materia

I've been grinding Limit Breaks for a couple hours now, had cover materia on the charactor that needed to do certain number of limits in the whole time i was grinding i got cover to level 2 (40%) chance of cover and yet it has not happened once,

Is cover materia broken in steam version?

Cover + counter attack works wonders for me.

counter attack usually screws me over (when getting enemy skills and such) so i just end up unequiping it.

Only most physical attacks will invoke cover. magic never does. Also, try unequipping, reloading, and reequipping it.
