That chest only has the materia in it during the Wutai sidequest. If you don't go to Wutai before you go to Midgar and the Sister Ray, the sidequest will never occur and that chest will perpetually be blocked by an NPC who only moves during a part of that sidequest. I don't know since I never skip that sidequest, often hitting it as soon as I get the Tiny Bronco, but everything I've read means you can't do it after disc/part 2 is complete.
Of course, this all doesn't explain why that woman couldn't just move her fat ass two feet to the left so you could loot the box anyway. Stupid scripting.
You're right, but Rufus and Reeve don't die (Reeve is Cait Sith)
I already knew that Reeve is Cait Sith (Barret calls him out toward the end: "You're busted, Reeve.") and that Rufus survived based on story synopses i've seen from the spinoffs; I wasn't aware Reeve had actually made it to safety though in the wake of Meteor; Cait Sith even says that there's no chance for the people still inside during the final cutscene, which made me think he was still down there somewhere with them. If he came back in the spinoffs, I wouldn't have known since I've never played them.
Understandable, but alot of people actually survive in midgar, especially those in the slums who are more protected due to the upper plates providing them with a roof.
