When you do the "+" you hold down the button and then press the next.
So its "hold down page up and then press menu (... V key is the default for menu )
But then again you can re-arrange every key.
X, S, T, R1+T, R1+S, X, S, T, R1+X, O, X, S, X. Thats the PSX buttons for it as well so you don't have to work it out, just convert that to what ever you have set up.

2. You can only get her final limit break after Tifa has... eh you can only do that in "part 2" of the game.

I guess this isnt a "mega spoiler" when you get Cloud back ( you will know when ) - THEN YOU CAN PLAY THE TUNE AND GET THE LIMITB REAK.
So, Yes, Cloud need to "go away" in the story and then "come back" before you can get it. Just progress through the story and you will know.
Otherwise you would be able to grind/abuse Tifa to get level 4 limit break already at first time in Nibelheim.
And looking at your Achievements (which I did) you can't get it at the moment, your not far along in the story!
You'll also need to have played the piano (any tune, doesn't have to have been this) during Cloud's flashback back in Kalm, when you first left Midgar. Tifa will comment on this before getting the Materia. Figured I'd mention it since it's easy to miss, and Elemental Materia are scarce in the game.
Prior to this, you'll only find the 1 gil.
