"An Unknown Exception Has Occurred"
The game ran fine for me until now. After listening to Rude, Elena and Tseng in the cave once I exit to the world map my screen goes black while I still hear the world map music. When I alt tab to the desktop I get an error popping up saying "An uknown exception has occurred" and the game crashes by itself.
I have tried exiting from both sides of the cave and have gotten the same results, it seems I can no longer progress with this error..

System Specs:
Windows 8
AMD Phenom II X4 955 Processor
Nvidia GTX 670
4GB Ram

Thanks but do you know if this will fix the problem because I have lost my progress twice because of this error and I don't feel like doing the same thing only to crash again.

I'm actually having the exact same problem. My second crash, I got into the cave and thought "Crap I forgot to save!" So I immediately turned around and left to save in case it crashed again, and ironically, it crashed again. I hadn't even gone through the conversation with the Turks yet.
Have you found a fix yet? Can't try the above mentioned link as I am at work; planning on trying it when I get home.

Ok, so I had exactly the same issue here and here is what I did and it seems to be ok now, I'm not sure which actually fixed it so here is the list.

Verified Game Cache Integrity ( all checked out 100% )
Saved outside of the cave
Switched party from Barret and Tifa to Aeris and Red XIII
Saved again
Entered cave - fought all random enemies and collected all items from various rooms
Talked to the Turks saying I know who they are
Got items from room above
Exited Cave from ENTRANCE - saved again ( the saves were more a paranoid thing just incase I crashed again )
Exited cave through back exit - no error this time.

System:
Win 7 Pro x64
AMD FX-8350 8 core 4Ghz
Gigabyte GA-990FXA-UD3 rev3.0
16GB Crucial Ballistix
ATI Radeon HD 7850 iTurbo 4GB
Samsung 250GB SSD C:

Same prolem : game crash sometime during any screen change (rarely) or using skill beta (most of the crash (90%))

crash can happen in any screen or any fight. Load the game and do it again will not crash again.

like during fight in goldsaucer battle room, ancient temple, wolrd map, ... etc

Still no solution ?

Having the same problem at the same spot. If I leave the cave from either end I get the error.

For some reason, the time it worked for me was the time I saved JUST before walking in the entrance, after hopping off the chocobo. Try that.

I am having a problem during the Hojo battle. I beat the first 2 bosses then on the 3rd when Cid is halfway through his limit break it freezes then crashes with the exception error message showing. Any clues as to what I can do to prevent this from happening?

This happened to me only once, and it seemed to happen when I used Beta towards the Midgar Zolom right after he used it on me.
