1. Counters with a physical attack when you're hit as if you selected 'Attack' however the chances of you countering depends on what level it is for example mastered makes you counter attack every single time you are hit and this goes the same for the other 2 Counter materias.
2. Pretty much the same as the above except you must link a magic or summon materia to it for it to do anything.
3. That's right yes, it's somewhere in the middle of the green light, just walk around and keep hitting your 'Ok' key/button and you'll eventually get it or you could just search 'counter materia' on google images and just look at the screenshot and check around the area that shows where they got it.
This one is the same as Magic Counter, except you need to link it with a command materia, Mime and Deathblow are the preferred ones to use. Mime particularly because you can mimic limit breaks or summons with it.
Side note about mimic + counter it'll only counter mimic once, so say you want to use Omnislash 8 times you'll need to fill a weapon and armor with max linked slots with Mimic and Counter IE: Ultima Weapon + Wizard Bracelet.
Thanks for the tip about mime+counter too.
Somewhat related this is the same for the 'Sneak Attack' Materia where if you pair it with a Magic materia it'll only use the highest magic on it.
