This is all i can think. It doesn't explain why you no longer have the Mythril though (if indeed you DEFINATELY had it in the first place and didn't pick the wrong chest).
I remember getting Aeris' limits ready as early as possible with the Mythril ready to visit the old man and realised i had to go to Gold Saucer and progress the story line before i could get the Mythril...
I already have the key, and completed Gold Saucer. It is definately my next stop, no doubt about it.
I have revistied EVERYTHING as well. Only one other person has had this problem besides me and he gave up.
Not good. I take it you haven't made multiple save files on your journey to this point...
Just find an FF7 save editor and plop either the mythril or Aeris's limit into your inventory (make sure the game isn't on when you do this). Boot the game back up and have at it.
Oh, and back up your save before you do this, naturally.

I can vouch for this guy. I remember recieving mythril atleast twice since i had no idea what was in the chest downstairs (turns out it was a gold armlet). Never heard of anyone else having this problem, but good luck fixing it. A save editor is probably the most reliable way of doing so.
