Can't use Limit Breaker Lv.4

I can't get the 4th lv. of the Limit Breakers.
For example: Barett have all his 2 attacks for his three limit Breakers and I already was at the point of the story where you are on the train which drives too Corel.
Same for Cloud (all three Limits Breakers and I was in the Battle Arena), Tifa... Yuffi... all three limit Breakers, and I'm at a point of the story where I should have the 5th limit.

So, why can't I use the fourth Limit Breaker? In the limit menu I can't set the fourth, the text is grey and not white.

Each of the characters has a certain item you must find to open the last limit break.

Like the Item I got for playing the song on the piano right? Well, I have it. or the item I got from Yuffi's dad... but still not possible...

Having it is not enough, you have to equip it to the character to unlock the Limit Break!

you need to have all 6 of that character's previous limit break in order to actually use the item (excluding vincent since he only has 4 total limit breaks and cait sith because he only has 2 limit breaks).
every level limit break has 2 different abilities you can use (for example cloud's level one limit break includes braver and cross-slash), and in order to get those limit breaks you need to either do the following: defeat 80/160 enemies or use that limit break 8 times total.
if you have all of a characters' level one limit breaks yet you still aren't on level two, that means you haven't killed enough enemies to unlock the next level limit break. if you have say, a level 3 limit break yet you don't have its second limit break, that means you need to use that first limit break 8 times total.
