The day performer Purple man helped into the suit appears dead inside the springlock suit two days later on Night 4. Now, THAT is the springlock failure. Purple man wasn't around to help the performer out of the suit and he had to take it off on his own, accidentally triggering the springlocks on himself and dying on the spot. The crew finds out and that's why during the birthday party there are NO employees wearing the suits, they've been deemed unfit for them.
During the party the teens shove the child into Fredbear's mouth, extending his jaws beyond their breaking point, which causes the head of the animatronic to come down crushing the child's skull to death. Two incidents involving the springlock suits, one in suit mode and one in animatronic mode get Fredbear's shut down and the suits decommissioned at Freddy's until they're repaired or deemed safe by the technician.
