I think it is just some obsession of the protagonist...

He was so obsessed with the painting (because the shadows on it) that he did reverse engineering to find out who was the painter (the man locked in the cage).

The protagonist isn't just depressed, he is nuts.

That's probably closer to the developers' intent, but maybe the island also represents his isolation. If you think about how the painting would have had to be painted, a man on a boat (assuming this island is far enough away from the shore) would be viewing it. Imagine being adrift and hopeless (such as a castaway on a boat) and the only land you see is uninhabited and contains no potable water or food. It's as if to say, 'it's all hopeless; even though I finally reached the shore, the shore is useless.'
Nice to see other peoples ideas, I personally never gave the paintings any attributes reflecting to the protagonist. For me they were just some background story, nothing important.

About the ending? The game tries to raise some awareness about how we fail to notice when someone is having serious problems and needs help. It also portraits how some of these individuals intentionally try to cover any sign of what he/she is about to do (There is a note somewhere in the game showing how to place books on the shelves withouth getting suspicious).

Back to my first 2 sentences. Thx for your theories, I just want to play the game again because of this, trying to look at each object and action in a more symbolic way :D

My art-house critique of the game might help, I cover the painting pretty extensively there :)

I explain this in my video: https://www.youtube.com/watch?v=jzZ0uwJoEts
The painting means nothing. Which says everything, really.

It doesn't mean anything. The protagonist is obsessed with it because he desperately wants to find meaning in it. Maybe this is an allegory for his life or something. He is driven crazy due to his obsession and In the end he gives up and kills the painter and simultaneously kills himself, giving up on looking for meaning in the painting and looking for meaning in his own life.
