So, you have to play it again if you want the achievement on a fair level.
There are some cheating ways to get it, including getting a savefile from someone right before the final boss to unlock it.
You can tell whether or not the glitch has affected your save file by looking at the color of the smoke behind the difficulty name in the file menu. One Life is supposed to be purple. If it's white, then you need to trash your save.
There is, however, one other thing you can do if you don't want to play it again. At least I know it exists, but I'm not sure where to find it or how to use it. I know someone who does, but they don't hang around this board anymore. It is called the Steam Achievement Manager. It is generally looked down upon because it can be used to unlock achievements that you have not earned, which is considered cheating, but it is acceptable to use it in situations like this because it can forcibly unlock achievements that failed to pop for reasons like this that are not the fault of the player.
Never heard about this one. Really.
But the difference between concluding a One Life game without dying at all and a One Life game with 1 or more deaths is that there will be a wisp of Ori's soul at the last death position, which will give you some of your lost ability points back when you collect it. This means, any run after dying at least once gives you a slight advantage over the very first run. And you get the Unhinged achievement only for not having this advantage on a One Life rerun and don't die. Look at a rerun with an existing wisp just as a training run. The wisps existence is why the savefile is "burned" and the achievement is permanently locked as you didn't really beat the One Life mode properly. This is at least what I know about it.
What is actually a glitch: if you don't wait the end-scene and credits to run out by its own, the Unhinged achievement may not pop up as well. You should never skip them.
