Sorry. It's the DOS version, not even the Windows version. Kind-of odd, right?
Wow.. the DOS version has the soundtracks from the Sega CD and Saturn version! *adds to wishlist*
This version is indeed the DOS verison, which has the best graphics, but it's missing a level, the Gamepad Calibration is absolute hell(Don't even try it), and the game runs through DOSBox, so it has a few bugs caused by emulation, and a few extra bugs because of it being for DOS. I seriously recommend not going for this version.
Instead, get the Windows version(Which you'll have to get on somwhere like eBay, but it's the most common -- and most cheap -- version, and it'll be well worth any long shipping periods, since it's basically the Mega-CD/Sega CD version, but with better graphics and sound, plus it's on the PC).

TL;DR: Don't get this version, get the Windows Version.
The DOS version is easier to get running under DOSBOX, rather than Win95 version

Which level is it missing?
Intestinal Distress.

Intestinal Distress, and the Windows version has Big Bruty, so the Windows version has two more levels than the DOS version.
If you decide to get the Windows version, let me know and I'll send you the link to a DLL that you can download to fix an annoying sound bug. :)

Sweet, I've got the win95 version from way back and I've never been able to run it properly because of that. Would you be able to fire that my way? I'd love to be able to play this again.
Sure.
Okay, so copy all the files from the CD to your PC, then download a new(Or newer) version of "WAIL.DLL" (Link at end of post)
Get this DLL, and place it inside the "ASSETS" folder(Replace the old one), and from now on, run the game from "WORM.EXE" in the ASSETS folder on your PC, and it should run smoothly. :) And yes, you will still have to have the CD in the drive to play the game.
Link to DLL: http://www.nodevice.com/dll/WAIL32_DLL/item19029.html
Note: You will have to unzip the DLL using WinZip, WinRAR, or something else that can unzip .zip files.

The worst imaginable, in my opinion. :tank:
Obviously you haven't played the Famicom, Game Gear, or Master System versions.
Guess they couldn't be bothered to get the Sega CD version.

I'd say it's more along the lines of the fact that it would take a lot of hassle to legitimately distribute a Sega CD/Mega-CD emulator commercially along with a ROM of the game.
Although, as to why they haven't decided to use the Windows 95 version, I don't know. My theory is that they couldn't be bothered to apply an official No-CD Hack.

That's actually fairly simple - the Windows 95 version was co-published with Activision.
I thought that Interplay now held the rights to all versions of Earthworm Jim?
