The many bugs of this game(Database)
So, everybody who has progressed a few hours into the game will notice that it is about as stable as a house built from oatmeal. My goal here is to keep track of as issues many as people can find, as I had a lot of trouble finding any solutions to some of these with google searches, forums, and discussions.
GRAPHICS ERROR - The most commonly referenced one, the issue of the game itself not launching. This has been answered already multiple times, but in case you came here before searching for it, simply open up your game directory\core\bos.cfg in notepad and change {display.height} to 600 and {display.width} to 800.
CLIPPING ON STAIRS, STUCK VEHICLES, UNABLE TO INTERACT, ETC. - The game does not have perfect motion mechanics, and as such it can be troublesome to maneuver. Be very careful with vehicles, as it is laughably easy to get them stuck, especially since they do not function well in turn based combat. Sometimes your character will be right next to something but will say unable to reach. Just move around a bit closer and make sure no squad members are near enough to possibly block an adjacent area. This solution also applies to being stuck on stairs, narrow hallways, ledges, etc.
EXIT GRIDS NOT WORKING - this is a very annoying one, as it renders your save game broken if you do not save frequently, This happened to me on the evereddy bunnies, brothers grimm, and bazaar day special encounters. The exit grid stops functioning once you LEAVE AND COME BACK to the area. According to the modding community, there is no fix to this - however, I have managed to make the bazaar day exit grid work again by shooting at the NPCs. In the brothers grimm encounter, the grid refused to work to work even after I shot and killed both mutants when I had to go back to my bunker for an additional medkit after finding the encounter. I am not very sure about this one, so please help me out with whether this is set to some encounters, completely random, or if it occurs on some missions.
SHOOTING THROUGH FLOORS/WALLS - This has happened to me on multiple occasions, and appears to still happen after loading or coming back to the map. The places I have discovered so far are: Fencing around the generator in the Quincy map - I discovered this one while trying to open up the door, and my sniper(who was on aggressive sentry mode) started shooting a deathclaw baby two floors down. Peoria - In multiple instances, I have been walking on the top ground area of the mission, only to find my characters shooting at nothing with the message log saying I killed a few cockroaches. Mardin - throughout the whole mission, my characters on aggressive sentry mode were constantly shooting at beastlords and cockroaches beneath the surface while on the top floor area. This mostly happened in the southern part of the top floor, but also occured elsewhere. The whole map seemed buggy. Osceolla - before the fight with Latham, there is a small area in the corner that the flamer mutant sits where the mutant in Latham's room is able to fire at you. Playing on insane difficulty, he killed me instantly at this range so I did not bother seeing if I can shoot back or if it is blocked.
RANDOM ENCOUNTERS - Not sure if this is a bug or not, but a lot of random encounters, especially raiders and reavers, will give enemies with guns but no ammo to the guns. They do sometimes spawn with ammo, but it is rare - most of the time I see reavers with miniguns, then putting the miniguns away and running at me with those threatening fists.
TIGHT SPACES - Human females are able to make their way through very thin spaces, often leading to unintended areas or even going outside the accessible map boundary.
SPECIAL ENCOUNTERS - This one is probably the most annoying - after finding all of the special encounters, one of two things will happen next time you are supposed to find one: either you encounter a duplicate and/or bugged version of the last encounter(only if on the same square as you found the last one), or the game will instantly crash(anywhere else, or if max amount of encounters have been found on above mentioned square). This IS fixable, as shown here: http://www.nma-fallout.com/showthread.php?192125&p=3737609 This is also exploitable. My last encounter happened to be the Pipboy, so now I have three other Pipboy encounters on the same tile, and after the fix, I can safely go through there without crashes.
PERMANENT KNOCKOUTS - Sometimes, super mutants will not get up after being knocked out. The only known fix for this is simply loading.
TOUGH GUY - The game will start randomly crashing with tough guy mode enabled.
Please help out with any info, corrections, ideas, fixes, answers, or additions to this database. Hope this helps for a more enjoyable time with the game!

I recently had a bug that ended up courrpting a file of mine : I'll post my story and how I posted here, if you don't read the entire story, I'll make a follow up post including the problem and then how to fix it.
So, I was playing as normal, I was in a jeep and had tried to go to the farmer encounter inside of the jeep.
It had spawned me into the encounter inside of my jeep, but ontop of a brahmin. I was stuck inside of turn-based combat between myself and the brahmin, with who were neutral. But here is where the first problem had arrised. When I was inside of that combat, the brahmin would skip their turns, and when it came around to be my turn, I just wouldn't get it. It would just go back to them and then they would skip, go back to them, they would skip, go back to them ect.
So, I saved my game as a new saved game, I didn't want to overwrite my actual game with a file that might of been broken. After I saved I quit the game, and when I had tried to relaod my saved game, any of them, it wouldn't allow me to load them. Not only that, but if I had tried to make a new game, and then try to save it wouldn't allow me to, it would just say "unable to save game" or something along those lines. And after I would quit to my Windows desktop Windows would give me a notification saying that bos_hr.exe (the game's executable) was courrpted.
I tried many things to try to fix my game, I tried deleting all (but one) of my saves, no fix, I tried uninstalling and reinstalling, no fix, I tried to update steam, no fix.
I found the soluton when I tried deleting my entire game folder (with my file moved out of course), and it wouldn't let me delete this one folder called "$$current$$". Whenever I tried to delete it it would say that the file cannot be opened or deleted because it was courrpted.
I rebooted my computer into safe mode (the only way to delete a file), I opened my games directory, deleted the file, verified the integrity of the game cache, moved my save back into my game's folder and then rebooted. And it worked! My save was able to be accessed and I was able to continue playing.

Well then, if anybody encounters that particular problem, you can now fix it

Follow up to my last post.
Problem : Games cannot be loaded or saved after an infinite turn base'd glitch (happened to me after spawning in on top of a brahmen)
Steps :
*Step 1 : Move your saved file out of your game's directory, (I would reccomend moving it to your desktop)
*Step 2 : Select your entire game's directory, and try to delete it.
If you cannot delete one or two files, continue to step 3
*Step 3 : Reboot your computer into safe mode
Step 4 : Go into your game's directory and delete the folders you couldn't delete due to courrptions.
*Step 5 : Go into your steam library, right click on the game's name, hit properties
*Step 6 : Under the local file's tab, hit verify the integrity of the game cache
*Step 7 : Boot into FoT, start a new game, and if it allows you to save, then you have fixed your game
Step 8 : Move your saved game back into the save game folder (after shutting down FoT) and
then load your saved game.
You're done! :D

@Slayer115
The HIDDEN ENEMIES one isn't a bug. Some enemies just have really high stealth... The same thing used to happen alot in multiplayer, someone with high sneak could be right next to you and you'd not know until they attacked.

TIGHT SPACES is a confirmed bug. All human females can squeeze into places that even a baby deathclaw or skinny ghoul can't reach.

@Simon Petrikov
If you find yourself stuck in combat turns again, just press ENTER to end combat, then go into options and set Turn Based mode to "Off (CTB)"

Here's two more bugs...
TOUGH GUY. This mode will make your game unstable and crash at random. I've tested this quite alot on several different computers over the years. Without tough guy I can finish the game without a single crash(seriously), but with tough guy I crash and freeze very often.

LAZY MUTANTS. I've only ever heard of this effecting super mutants. Sometimes they'll take a knock to the head and wont get back up... they'll be permanently unconscious. I don't know of any fix other than loading or killing them.

I had tried everything to get out of the combat, but nothing had worked. It wouldn't let me change the combat or end it in any sort of way.

In the St Louis missiln, there is a glitch where the exit grid will not appear.
You must have Talon squad in the APC with you
You must then drive the APC to the exit area (you can hit M on your map, and it'll be the A circle).
Drive it close to the building and then you should be out of the mission.
Papa Shekels Jul 10, 2014 @ 9:15am
That is the mission objective. You have to heal the injured squad and escort them out.

I did, that wasnt the problem, the problem is that the exit grid will not appear till you get very close to it?
Hopper Jul 10, 2014 @ 12:10pm
The mission is just designed that way, you have to get the troops to the escape area to enable the exit grid.. It's not a bug, but I see how it could cause confusion.
Papa Shekels Jul 11, 2014 @ 4:26pm
Yeah, if you read the briefing carefully, your objective is to bring the squad over to the extraction point. So naturally, the game does not bring the exit grid up until you get near the extraction point with the squad so people do not skip through the mission. Knowing how buggy this game is, the exit grid would probably be there upon entering the mission otherwise.
󠁳⁧⁧Dude Jul 11, 2014 @ 5:54pm
The grid isnt there when you go into the mission, it only activates after you've completed the two objectives, but it doesnt show up till you're close enough to observe it with a magnifying glass.
And even if it isnt a bug, I feel as if it would be safe to keep it here incase people aren't confused like I was.
Papa Shekels Jul 11, 2014 @ 8:20pm
Well, the objective briefing, the commander's dialogue, and the pipboy map all tell you to go there, so not many people will be confused about it.
