How do you level affiliations?

By playing poker.

The only time you don't get points for your affiliation is when you haven't signed up to one or you need to level up by going into the affiliation tab.

Level up by going to the affiliation tab? I don't see that option in the affiliations. That doesn't appear to be the the results I'm getting. I'm level 82 and have poured hours into this game for the past year and have spent the time aligned with some affiliation - and I'm nowhere near max level. Do I have to be wearing affiliation "colors" to be getting rep with them?

Press the Rank Up button on the affiliation screen. Also you may be maxed out on rep earned from single player - you'll have to play multiplayer to earn more rep.

I'm looking at the affiliation right now and I don't see a Rank Up. I've only played single player for an hour or two and I've got 148 hours in the game on ranked tournaments and multiplayer ring games.

You're not max level in any affiliation. As long as you're below level 50 in your current affiliation, you can continue to earn rep, and gain levels, in that affiliation. Once you've reached level 50 in an affiliation, you can rank up in that affiliation. For example, you're currently level 20 Hearts Rank 1. When you reach level 50 Hearts, you can press the Rank Up button on the affiliation screen to go to Rank 2. You'll then be level 1 Hearts Rank 2, with another 50 levels to go, along with the associated affiliation rewards, to get to Rank 3. Max rank is 5 for each affiliation. You're currently level 25 Clubs Rank 1, level 15 Spades Rank 1, and level 13 Diamonds Rank 1. You're 20 Hearts + 25 Clubs + 15 Spades + 13 Diamonds + your original 10 Tourist for a total overall level of 83.
