Its a game about plague. People die in horrific ways. Violence is blooming. There is also some plot-related nudity. But be assured - all graphic content is added with purpose. Its all plot driven and not gratuitous.
* strong language (not a lot, but the game includes a number of words that would be beeped out here on Steamcommunity, for example), including children swearing;
* child abuse (non-graphical; however, the very peculiar role children play in the Town's ecosystem is a major plot point. Children live without parents, trade in weapons, and you get a quest to kill a child, for example, that you can accept. There's a backstory involving kidnapping. I'm not sure what exactly constitutes child abuse, bit it's something to keep in mind);
* gore/violence/blood (I don't find it very graphical—less so than in some other games that include fighting mechanics, but it's implied. There is non-graphical organ harvesting, but there's no gratuitous gore, although, you know, there's a plague going on, and people are dying);
* as for nudity, I'm not sure! There is a caste of ladies who wear revealing clothes like these (the screenshot if from the non-enhanced version, just for you to see the general idea: http://www.igray.ru/images/reviews/mora/19.jpg ). If you see nipples there, then I guess there's also nudity. No sex scenes at all. No full frontal nudity. Several vaguely erotic text scenes and some innuendos;
* abuse/domestic abuse (said half-naked women are also actually half-slaves of their "husbands"; it's also implied that, since the local religion includes the worship of the body, they also offer religious sexual services. It's talked about in religious context rather than in sexual one, and it's totally text-only).
Some things that seem to be less major:
* alcohol references (rather abundant, including several alcoholic characters portrayed as likeable);
* substabce references (I find them tasteful—there's an outbreak of a deadly disease, for crying out loud! You are encouraged to take morphine; there's a drug addict portrayed as likeable; one minor text scene includes straightforward offering substances to a minor);
* suggestive themes;
* tobacco references (a couple of NPCs are smokers).
I think this is it.
I don't know what the technical rating would be, but, as for me, the general idea of Pathologic goes like this:
This is obviously a game intended for adults. However, in order to realize the horrendousness of your situation, you have to be immersed into the plot and events. So if you're, for example, concerned that a minor (or, say, your parents) enter your room when you're playing, catch you by surprise and end up shocked/traumatized, I'd say the chances of that are rather low.
