Controller Disconnects and game Freezes

I have been having this problem while in game, on both online and local where the game freezes and the music keeps playing. Sometimes it is for a few seconds and other times it keeps on indefinitely forcing me to sleep my computer and log on again(this since alt-tab wont work). I also noticed that when I reopened the game my controller appeared to be connected but not working. I am using a wireless Xbone controller connected by wireless. I really like playing this game but the more I play the more it keeps happening. Hope someone can help me.

Try updating your graphics card drivers from your manufacturer's website.

If you think it's a controller related issue I'd also double check that in your options controller mode is set to 'Xinput'.

Testing some more with a friend I found that the Xbox one controller connected by Bluetooth is what made the game freeze; it happened in his game too. I have an Xbox 360 controller and it doesn't have this problem. I had the settings already at Xinput and then tried Dinput still I left it at Xinput and it doesn't seem to work. Thanks for the suggestions I will try that too.
