If you happen to see no fish or only the slow rotten ones, go back to the house and talk to Cid, that will trigger the respawn of the fishes. Keeping him alive just makes one more useless NPC that you have no reason to talk to ever again after leaving the island.
Depending on much in hurry you are to progress in the story, you want to kill off Cid since depending on your luck, it can take a while to get enough good fish for him to recover.
When Celes tries to kill herself. If you choose to save Cid, this type of character development doesn't happen.
