Sally Smithson Analyzed
NOTE: Everything written in this post, minus quotations, are my own personal conclusions I have made based off studying the lore (both purposeful and accidental) and playing the game.

Sally Smithson
I just want to quickly mention her name and why it seems so dull and Jane-Doe-like; and it's probably because her name isn't very important. Unlike the Macmillans or Thompsons or even Ojomo who all have strong names, Sally isn't defined by her name. "Smithson" is not her maiden name, so we know her only by Sally. Later, though, she'll take on an even more prominent and important title, one everyone will know her as: nurse.

This could be a big clue -- the Entity could have had a hand in Sally's life earlier than we may have initially thought. Once they moved to the aforementioned town (I'm assuming the town which housed the Crotus Prenn Asylum) everything went to ♥♥♥♥. Perhaps the Entity was there, lying in wait, searching for someone to prey on. Then Sally arrived.
Andrew worked as a lumberjack - a job with its perils. And one day Andrew’s foreman had to pay Sally a visit, forever changing her life. She was alone.
Now, completely isolated, like our other killers, her mind and willpower become vulnerable enough for the Entity to step in and begin to pull her strings. The Entity works devilishly, first attacking the things our killers once loved and then forcing them into solidarity and drowning them in loneliness.
Without food on the table, the only employment she could get was at the Crotus Prenn Asylum. Nobody sought employment there unless they were in dire need. Just like Sally. Without any education, she started at the bottom and after two decades she hadn’t progressed any closer to the top. Instead her mind had reached its limits. Two decades of seeing horrid things that violates the eyes. Memories that are re-played every night. Being abused verbally and physically, by people without limits. Sally saw insanity from the outside, just to catch it herself.
Let's assume that Sally is around 20 when she is forced into working at Crotus Prenn (since she didn't have any education, children, and was newly widowed) and ended up being around 40 years old by the time her mind finally succumbed to "insanity". Since it's an insane asylum, the Entity probably let nature take it's course and threw Sally to the dogs: causing her husband to die, then leaving her to fend for herself at the asylum. The Entity knew she would break, though it probably did not think it would take her 20 years to do so.
Finally she could not take it anymore and concepts of purification emerged inside her. She did what she felt was necessary. As the morning staff arrived one day in September - they found over fifty dead patients, lifeless, in their bed. Sally was the only one at the scene, rocking back and forth. Exactly what happened is only known by her, but it seems that some of them had been choked as they had marks around their necks.
So obviously, Sally murdered her patients in their sleep. The happening was ruled as a mass-suicide and the Asylum was prominently shut down:

Having devoted the last 20+ years of her life to nursing, her mind was completely before her, and she continued to walk the halls, searching for patients in need of her assistance. She was unable to accept the fact that her life as a nurse was over. Until one more person needed her "help"...
Finally, one night, smoke rose from the woods as Crotus Prenn had been set ablaze. The bystanders did nothing. They just let it burn.

The Nurse
Finally, as the Asylum was burning down, Sally's worst fear was coming true: everything was ending. Her Asylum, her haven for the sick, was turning to ash before her. This is where the Entity comes to her, and offers her eternity. It promises her Crotus Prenn, and promises her more patients to purify.

The Nurse is the only killer (so far) that willingly does the Entity's bidding. Her mind, much like the minds of Crotus Prenn, is twisted beyond repair. She sees survivors as sick patients that need to be purged and purified of their illnesses.

The Nurse and Nea
This killer is the first to have a direct connection to a survivor. I believe that the Nurse and Nea were taken by the Entity at the same time.

Nea "went too far" when she, instead of just tagging the Asylum, decided to up the ante and set it on fire. She did not realize the horrors that the Asylum had seen, and the power bubbling up within it's walls. The Asylum, alive, muddles Nea's mind and she becomes lost in the labyrinth of rooms.

This is where Sally hears her coughs and cries for help and finds her. I say Sally because I think, initially, she intended to treat Nea as a patient and either keep her in the Asylum or lead her to safety. That changed, though, because when the Entity offered Sally an eternity at Crotus Prenn, it needed Nea as well.

Sally tries to catch Nea; but Nea is too quick on her feet and Sally only manages to rip a piece of her flannel in the heat of the chase. Thus, returning to the Entity with only the single shred of clothing and no Nea, the Entity strangles her, taking her last breath as she had previously done to her patients. The way she floats through the air, a pillowcase covering her face, her sporadic and ghostly breathing all point to signs of strangulation.

Appearance
Following through with the theory that the Nurse found Nea, she could have suffered severe burns trying to rescue/capture her. This would explain why her body is clad in bandages:

The Pillowcase
Why is her face hidden? Unlike the Trapper, I don't believe the pillowcase was a personal choice. I like to think covering her face is a type of eternal punishment from the Entity for not willingly giving her Nea. Also, it takes away any personal connection that she may have with her patients (the survivors) since she's nothing more than a pawn for the Entity now.

The Warden was one of Sally's many victims. Superiors have that uncanny knack for seemingly being in many places at one time, with eyes everywhere. After taking her, the Entity made this phenomena a reality.

Add-ons and what they represent

So, I didn't go through the add-ons for the other killers because they are pretty self-explanatory. The Nurse's add-ons are, however, very interesting and tell a story of their own.

Bad Man Keepsake and Metal Spoon: The "Bad Man", patient of room 202, used this metal spoon to extract the tooth from either himself or his roommate, and kept it for good luck.

Wooden Horse and White Nit Comb: These were both the symbol of deep connections between patients and their nurse, who I believe to have been Nurse Moris.

Pocket Watch: Taken from Nurse Moris' body, a coworker that Sally was jealous of due to her ability to calm and soothe her patients with small trinkets.

Matchbox: The one Nea used to burn the Asylum down, it wouldn't have any meaning otherwise.

Torn Bookmark and Dark Cincture: When Sally tried to bring up ideas of purification to the priest (Father Campbell), he rejected her ideas and she ripped the bookmark of a bible. Later, she would go on to take the cincture of his robe and perhaps use it to strangle him and others with.

Anxious Gasp, Spasmodic Breath, Heavy Panting, Ataxic Respiration and Fragile Wheeze: The last breaths of many of her mentally and physically ill patients.

Plaid Flannel: Ripped off of Nea's flannel as she was trying to escape the burning Asylum.

I too thought that's how she died when she was first released! It makes sense if you look at the way she carries herself and how her head is always half-♥♥♥♥♥♥ as if her neck was broken (plus her troubled breathing as you mentioned). However, I couldn't see that fitting into her timeline very well since the lore says that there were rumors of a woman still living in the asylum after it had been shut down. Looking at her Mori and the whole "the last breath" theme I'd safely bet that she was taken by the Entity through strangulation. Hanging and strangling have similar effects! :theskull:

I think she hung herself, she floats and doesn't walk and if you see her legs they are tilted to the side.

She could have worked with soldiers from WW2. She is from the 20th century (1901-2000) and WW2 was 1939-1945.

The gas mask was invented in 1915 (well within her timeline); I expect the asylum she worked in provided them to staff in the event of having to subdue/treat patients.

She could have also used one to navigate the asylum while it was burning down while trying to find Nea.

Gas masks like the ones she bears (WW1) have a limited lifespan and it could also explain why she has such troubled breathing, if the ones she used were expired or contaminated.

I really love that they gave her the gas mask -- a plague doctor mask would have been cool too but that would be lorebreaking since she is 20th century (plague doctor 17th century).

So do you think Sally hung herself and she was a form of ghost chasing Nea? Or was she litteraly rocking baxk and forth in a corner?

I don't think Nea and the nurse met before the entity, because they would have lived in different time periods.

The nurse is from the victorian era, since her dress is identical to the dresses from the victorian age, which would put her somewhere between 1837-1901. Since she's wearing the dress in the realm of the entity, it would prove that her life on the material plane ended within the victorian era.

However, Nea obviously isn't from the victorian era, since she uses graffiti to write her nickname 'Mashtyx' on walls in her lore. The first spray can was patented in 1929, but she also has more modern things: a beanie, a chain in her pants etc.

TLDR: The nurse and Nea couldn't have met in real life, because the nurse must have vanished before 1901 and Nea's graffiti cans were only invented in 1929.

But the nurse has a WW2 gas mask as customization so she can't be from you time periode
