The killer instinct works with a terror radius, not whether a survivor can hear it or not.
Plaything and Hysteria make the survivors oblivious, but your terror radius stays.
What you don't want is something like Tinkerer, which will completely suppress your terror radius.
oblivious supresses your terror radius, no matter what you use, be it hysteria or plaything or etched ruler or whatever, and any surv that isn't affected by your terror radius is invisible for killer instinct
Other instances such as Pyramid head's trails, Pinheads Lament configuration, the twins power, and the artist, those all still will trigger killer instinct.
If none of the things I said ring a bell, you might want to make a bug ticket.
Oblivious does not suppress the killer's terror radius, you just can't hear it, but it's still there. As long as you are in it, it does not matter if you are affected by it or not.
