A Nurse's Calling can make Sloppy Butcher a great perk to have, but not every killer has the mobility to make it work.
You say that as if all killers had that choice. Some killers only get to apply worthless things like blindness.
Sloppy was quite good on a number of killers and was great for those who loved to tunnel.
Generally speaking I play against lobbies who have a guarantee of 1 brand new part/instant heal.
Overall most teams I play want to bring 3 items to I bring Franklins. Nullifying surviovr items is in fact quite powerful.
Medkits still strongly outperform it
Use quad slow or default corrupt painres pop + bamboozle/aura/no way out/noed
Thats all u have to use as a killer nowadays
and 90sec of mangled is still quite long if you inflict it every single time you attack as a M1 killers
its a decent slowdown, its just right now, either you tunnel your victims, or you focus much more on survivors who are inflicted by mangled, but keep repairing gens
While the nerf is quite strong, the perk is still plenty viable, now you have to be more speedy, and decide of who you focus (tunneling or survivors who delay heals)
Anyway peoples still gonna take sloppy butcher, since its still a good perk, still a general perk, meaning any killer can learn it from the beginning, and you don't have much good other perks anyway until you buy other killers, so beginners will keep using it
