If he standed twice, check if there are other perks/objects involved.
The only time a survivor can unhook themselves on second stage is when the anti-face camp bar fills up completely which honestly shouldn’t ever happen if you’re playing killer.

The thing is.. if the survivor is on the hook and goes second he carries over any anti camp progress he had before. So if you camped him into second phase and then you were around just a little bit more he can unhook himself even in second stage.
But... that only happens if you camp....sooooo.. If you stay away at least 16 m you are good to go and usually other hooks are also more than 16m away. So I suspect you just camped a little too much
The anti facecamp bar fills up quite a bit for certain locations?
Or I’ve noticed it go up in the basement a lot for killer to go up and down stairs, bringing someone back, or sit above the hooks ?
