Shrine of Secrets is not random. All perks appearing are chosen by the devs.
The developers have always been claiming that they roll an RNG to determine the perks in the shrine, but that is a complete lie. They always hand-pick which perks appear there.

As the biggest example and evidence, as soon as the Skull Merchant archive tome released, developers realized that most people never bought this awful DLC and would likely not be able to finish most challenges that require specific perks.

Because of that, there has been a Tools of Torment perk in the Shrine for 6 weeks in a row. And not a single time prior to the tome's release. Every single perk from that horrible DLC has appeared exactly once.
You don't need to be a statistician to realize how unlikely that is.

Deadlock, the most top-rated slowdown perk, has not appeared in the Shrine ever since its release several years ago.
BBQ&Chilli has never appeared until its bloodpoint bonus has been removed.

Next time you see a free perk, like Adrenaline, being put into the shrine, remember that the developers intentionally put it in there to screw you over.

This week the Shrine contains 1 licensed and 3 free perks. The developers for some reason CHOSE to put those 3 free perks into the Shrine, knowing that nobody would ever buy them.
And the reason for Mettle of Man being there is the related challenge in the current tome.

They weren't determined by RNG, they were specifically chosen by the devs. The Shrine is a complete BS that doesn't fulfill its original purpose (giving access to licensed meta perks without having to pay real money).

Not sure where you get that RNG take, but they've been adding perks from the archives (during new tomes) for years, they also make "community choice" shrine from time to time. It's not a secret.

My point is, if it's clearly not random, why do developers add free perks to the shrine as a useless clutter nobody will ever buy?

Because they want you to buy DLCs, of course. Shrine is outdated thing for perks from the archives, basic perks and rarely licensed ones. I'm pretty sure they always wait a year or so before adding a licensed perk.
