Hell, I miss it on occasion. If you're honest, chances are it happens sometimes to you too. Maybe you're distracted by a sudden heartbeat. Maybe your phone rings. Whatever.

However there are people that no matter what blow up the same generator 4 times while the killer is hooking someone, at that point I usually just leave and find another one across the map and wish them the best of luck.
If the killer is off hooking someone it's entirely possible that they're blowing it on a nearby generator INTENTIONALLY to try and bait the killer away from the sacrifice to attempt a rescue. I've done that with some killers who've been shown to camp. Works... sometimes.
90% of the time they are newbies that don't have any killer detection perks so they're the ones that get caught.
This is also something you can do, but fast vaulting a window/pallet works better, you can do this near a generator to make the same kind of alert but without screwing the generator progress

Egad. I wouldn't do it on a generator that had any real progress. Nor would I do it had I not just recently started fixing one and noticed a nearby hooking. It's one of those "moment of opportunity" things.
I also find it can work a bit better as bait (though still obviously hit or miss), because it seems a little less like it's intentionally baiting the killer away for a quick rescue, and more like you messed up and are ripe for the picking.
I just booked it outta there.
The good part is that you still get the points for the work you did.
that said sometimes its just due to ♥♥♥♥♥♥♥♥, like when i decide to stop fixing the gen to do something else it just randomly blows even doh i didn't fail any check jump back on it again instant BOOM
the trick to getting off a gen without it sparking is to tap M2 before getting off, hope that helps you out
