yes, once you die with a medkit or a map. it's gone. you need to unlock it again, or find it in a chest and survive.
Honestly, this whole "Get items/addons/offerings from out-of-game, then bring them in for an advantage" is an incredibly poor design. It can make games incredibly lopsided just because you recently found a nice item in your blood web. Yeah, it might last for only one game, but it's not like that player is doing a best of 3 against you. You're still potentially screwing his game with your offering/addon.
Since most of the killers I pair up against are Wraiths, I honestly don't feel sorry for them.
