What's the nastiest doctor build
Merciless storm, hex: huntress lullaby. unnerving presence, and throw in an overcharge just to top it off. Any ideas for doctor builds?

I've had doctors do really well with starstruck, they just hook then zap and know where people are within the range and down them in one hit.

skill check perks are pretty much just for memes, survivors just won't often fail them unless they're brand new to the game. Doctor is great at locating survivors, and has some anti-loop if you can time your shocks correctly at vaults and pallets, so the things he really lacks most are lethality and gen defense. Your shocks can interrupt people off of totems, so hex perks can be attractive. You could also run obsession perks, since you'll pretty much always know where the obsession is.

Even with the best build though, Doctor is still just a basic M1 killer that will struggle to do much of anything against really good survivors.

Increase your terror radius with Distressing, then use whatever perks benefit from a bigger terror radius, like Starstruck. Could also use Nurse’s Calling paired with Coulrophobia and Sloppy Butcher.

Whatever you do, do NOT use Monitor and Abuse on Doc: it’s trash and better suited for speedy/ranged killers. Keep your terror radius as big as possible for best value of your power.

Edit: The Onryo’s perk that causes difficult skill checks to appear at 90% gen progress is also amazing on Doctor since his madness effect also applies to it, meaning the skill checks will pop up in different spots and in different directions.

A lot of those seem attractive at first glance but aren't super impactful in practice, except maybe Sloppy. Distressing is more something that feels nice on doctor rather than something that'll have a huge impact every game. Similar to using Iron Maiden on Huntress or Trickster. Nurses Calling isn't great either, Doctor already has really good map vision pretty much at all times. And Coulrophobia, even if they all have items, it'll maybe earn you a few extra seconds per match. If you want to punish items, Franklins can be a good choice instead, and will actually make them greedy to come back and grab an item they dropped. Thanatophobia is good for slowdown as well.

lol definitely, it can be a crazy game for survivors against a doc with this build and offering.

I like the starstruck builds. I wonder how effective a Play With Your Food + Nemesis type build would be on doctor, has anyone tried?

His anti-loop kinda sucks to be honest, too unreliable because of latency. Ping will cause survivors to already have vaulted before your shock comes out even when it appears on your screen as if you were early enough.

It's not great, I agree, but it's at least something.

Until get new perks, i do ok with...

Calm + Discipline

DeadMan's Switch, Discordance, Pop and Jolt.

I use blast to locate, addons for chasing and perks for gens.

The only winning doctor build is : not to play Doctor.

He has weak antiloop and strong survivor detection - that's the opposite of what you want.
I miss when Nurses Calling detected survivors trying to leave madness 3.
