If they're running it's just the occasional pool of blood on the ground and doesn't stand out very much. If they walk or stand still the blood gathers and becomes a bit more discernable. It just takes getting used to to use it reliably in conjuction with other methods of finding people.
Sloppy Butcher increases the frequency and size of blood puddles on the ground, and Bloodhound makes them glow bright red so they're easily visible and last longer to boot.
Edit: Sloppy Butcher also makes survivors bleed out on the ground faster.
