I'm pretty sure it does

The description of the perk doesn't have any stipulations about a survivor being injured, or a survivor who is in the dying state. It is simply: "Grunts of pain caused by injuries are reduced by 50/75/100 %" one would assume that would mean, at all times, at least that's the implication from the official description.

I don't see why it wouldn't.

Are you sure? I don't mean to be stubborn but I really do think it does. Either that or I haven't been paying attention enough for the past three years and Iron Will is a perk that has been in the game since launch.

I don't run it too often anymore which is why I can't completely remember, but I really do feel that it does.

I'll take your word for it then, I'll have to run an immersed build again soon just to double check for myself then lol, it kinda sucks if it doesn't work that way.

not at all, it's only work when you're injured and not in dying state.
ACE > Iron Will

Iron Will doesn't reduce grunts of pain while downed, only while injured. On the other hand, No Mither reduces grunts of pain at all times by 50%, including while downed.

No Mither reduces sound when downed but I don't think people run No Mither for sound reduction

True

Maybe that's why I don't run Iron Will as much as I used to, cause I'm an Ace main

Iron will does nothing in drying state. Because according to devs, dying =/= injured.
Only in injured state you hear literally nothing. Would be annoying as killer to find a no-sound slug anyway.

Iron will only effects you in Injured State <--

Dying state is separate from injured so it does not work [Also this would annoy killers looking for survivor's in grass.

As others have mentioned, Iron will does not work while slugged.

However, with the current game patch, Survivor sounds are sometimes bugged and they make no sounds, thus getting a free iron will AND silencing their sounds while on the ground. It also can mute locker 'breathing' sounds.
