I got a 4k with pigs unique perks only and I didn't get the achievement, twice.

You're using an ebony mori. Odds are you aren't earning enough points to get "Merciless" in that manner. Try hooking survivors twice instead of once and see if that works.

Trust me, I lost adept because I didn't earn enough points more than once.

4k has nothing to do with the achievement (sadly) it has to be a MERCILESS VICTORY and the requirements for it are no longer a mere "4k" but a multitude of objecitves

I believe you need 16 points for a merciless victory, so hook each survivor twice, then mori them.

Survivors simply have to escape as far as my knowledge goes.

Off the record, you can still earn adept even if there's a disconnct in the game, or if you start the game with 3 survivors, but you still need to basically earn enough points to earn the requirement.

The current system made it harder to reach merciless victory so the adept killer achievements are better done in lower ranks :(

It isn't about points. You just need to kill all of the survivors without using a mori. If someone disconnects, it still counts as a kill towards the achievement.

It's about points while making sure they're all killed. I killed all survivors as Doctor within 5 minutes running his unique perks, but because I wasn't 'Devious' enough, I didn't earn adept.
I had only brought some survivors to madness 2 because they were downed so quickly I didn't even care if they got to 3 or not. I thought it was about kills. Turns out it's more than that.

Adapt can be achieved through ruthless killer as long as all four survivors die. It is based off the original Merciless meaning when how many you killed was the Disapointed, Brutal, Ruthless, and Merciless rating.

You must get at least 5k or 6k in the Brutality and Deviousness category, and at least 4k in the Hunter and Sacrifice category, with the character's Perks.
Mories are allowed.

By my opinion, you've had 8k in the Hunter category, but less than 5k / 6k in the Brutality or Deviousness category.

Tried the same with Legion, but different circumstances.

• Started the trial with three survivors, one disconnected in the middle of the match, sacrificed the other two remaining survivors
  
• Got the "Merciless Victory" message
  
• Three emblems were Iridescent, one was Gold
  
• Got 16k BP
  
• Used a BP offering (so I got 32k in total) and two addons
  
• Didn't get the achievement

I'm not sure what to make of this. Seems like it doesn't depend on the victory message, which makes the achievement's description misleading. It would be easier to know how to unlock this achievement if we'd know the actual requirements... but dis is DBD, ain't gonna get changed until a year later ;D
Are addons/offerings even allowed for this achievement? I mean, the description says specifically only with the killer's unique perks, which could mean "no offerings and addons" unless someone can confirm that this is not the case.

Add-ons and Offerings are allowed.
If you use the Offering(s) to get bonus Bloodpoints, marked as a post-game bonus, then it doesn't count.
The ammount in each category counts. It is, just like I've mentioned, 4k in each category, and then 5k / 6k in Brutality and Deviousness category with four Sacrifices / Kills.

And, I have been surfing through the intenet, and I've found out, that if you manage to get two emblems if Iridsecent rarity and two other emblems with Gold rarity, you should get the achievement, if you Sacrifice / Kill three Survivors.

Yes, and to get this achievement, the match has to take longer than 2 minutes.

Offerings and bloodpoints have NOTHING TO DO with the victory result, that was in previous systems

With survivors... Don't escape out the hatch or it won't count. You need to exit a gate.
