Yes, the ww2 nurse costume has been retroactively added to the bloodstained sack DLC. Those who had already bought the sack, got the nurse costume immediately.

(If you have bought the sack and didn't get nurse costume, try verifying game files)

It was only recently added to the Bloodsack.
