I never use flashlights, but it does seem you need to be really accurate with it. For a start, you need to be at the right range. You could be too far away, despite the fact it looks like it's hitting the killer. It also seems being too close to the killer stops it from working? I'm not sure if that's a thing though. It also takes a bit of time for it to properly work. If you're using a common/uncommon torch, there's a fair chance it'll take a bit longer for it to work on a killer.
Generally, I'd say only use the torch when the killer is locked in an animation to guarantee a hit, to guarantee a fast save, or a clutch save on a survivor, but the killers don't tend to appreciate that "exploitation", if it counts as that.
