you have to play with your break between 2 and 3 to handle this. but i've also never had a obscure "timer"-problem while reeling a 8+ pound unique peacock only with break 2.
@abstryx, your fish wasn't strong/heavy enough to rip the mouth. just try it again with trophy peacock or brown trout to make it happen.
There should be some mechanic to allow fish that were merely snagged/poorly-hooked to escape because the fish spit the hook or it tore out. Likewise, it should be possible to hook the fish so well that no amount of low-tension or high-tension would let the fish get away.
What kind of hooks are used in this game..it has to be like to top of a coat hanger, if they can "slip off" like that..here's an example:

Drag setting 3 -fish runs line from reel but lost from high tension, sometimes at yellow, sometimes at red. Hook size doesent matter #1/0 or #4/0 makes no difference.
Drag setting 2 - fisch runs about 100 meter, reel it back in and lost for no reasons at around 70 meter (must be the timer).
Or fish runs towards me - lost from low line tension. Despite low tension it does not reel in. Red hook indicator lower end of tension scale while tension meter is almost full (yellow 4 bars).
I must keep reel in button pressed at all times, if i release button all gets red instantly and fish is lost, unreasonable.
Compared to rl fishing high and low tension lost happens waaaaay to often in game. Tension meter often is stuck at 1, 2 or 3 bars. Had instant rod breaks out of nowhere. In CBT there has been symboles for force on reel and rod beside the tension meter, i never have seen them in early access, removed or bugged?
Fish "fights" are more challenging now but i get all this adrenaline for the wrong reasons...
I feel that stuff needs to be worked out in the next update if the update shall be about exciting fights. The adrenaline needs to come from a hard fight with locked on controls and indicators and it must be my fault if i loose the fish. Adrenaline from lost due to strange unreasonable indicators and controls is not the way to go.

Agreed. Drives me nuts when this happens.
Without proper tension or a (Tight Line) the hook can literally fall out of their mouth. So I think they have to make this a mechanic of the game.
Line strikes happen in real life as well as bad hook setting. A fish in real life can throw a hook, just happens way too often in this game. I agree regarding the "magically rips its mouth off" though. If the line test can't handle it, it should break. If it can is another story. Keep those hooks sharp!
Tested a 2" Worm on a #3/0 jig: Clearly an improper setup, the small worm on a large hook:
Results:
- The Young Largemouth were on the lure almost every cast and swimming to the bank still trying but failing to get hooked on it.
- Hooked a Largemouth that escaped from High Tension quite easily (my first ever High Tension escape ~ I assume because I don't usually fish with such an awkward lure setup)
