How do I even start the giants quest?
I've beaten the cosmic tree several times, and the holy pig quest, and the god quest, etc. The game keeps telling me I've failed the giants quest... do I have to side with the orcs or something?

It doesn't matter who you side with. You can continue the game after you beat the cosmic quest, it should start shortly afterwards. But even if you don't beat it it will happen eventually, you just have to wait till it automatically starts.

Kicks in automatically, via a number of different starts. oracle, deity, wandering herald, wounded guy in a footprint etc

If you're getting grounds tremors every moon then its already started.

Had the ground tremors for over a hundred turns without anything else happening, two games in a row where I just gave up and started over.

The RNG means there can be a large range of turns before it triggers. The devs have said that finishing the Cosmic Tree quest helps, but won't automatically trigger it.

I had a game where it didn't trigger until after turn 200, maybe it was like 220?

Edit to add: The ground tremors didn't appear to have any relationship to the Giants quest actually starting. They just fired off constantly loooong before the Giants quest started for me.

Finishing the divine quest line seems too trigger it consistently within a few turns assuming its not already started, however that may be just coincidence on my playthroughs also worth noting i'm usually serveral hundred turns in by that point.

The ground tremors was more a way of checking it (dlc) was active vs other random events like bloodmoon warewolf attacks which seem to be replaced by constant tremors instead. Which are then upgraded to attacks of differnt kinds depending how far along the questline one is.
