Are you defenders marking/pace/acceleration low? That could also be a cause.
One of the key factors of making a formation hard to beat is playing narrow. Narrow condesnses your back like making it extremely hard to penetrate particulary through the middle. This is very useful if you have slow, but very good in the air CB who can just wait for the crosses to come in.
Finally check your opposition instructions. You dont want to mark someone tight if they are fast as they will just turn and break away.
Thanks for the feedback, appreciate it. Tried narrow and no marking on the fast forward line. Still lost but not to the insane breakaways. Now they just played the ball around the box while my defenders stared in awe (Skrtl & Sakho). Still, this is something I can work with so an improvement there.

Of course this is only theory crafting only done one season.
I'm on fourth season now as Man Utd. Using De Gea as my only GK on whole season I never get conceded more than 17 goals per season. My DF are Shaw, Smalling, Jones, Darmian.

this version is better than previous years, but it still needs a lot of work. Hell, my goalkeeper just made a save, spilled the ball about 2 feet in front of the line. No one was near him, 5 seconds later as he gets to his feet he kicks the ball into his own net for an own goal. Seriously? Newcastle are crap, and regularly implode... but never quite like that
The save tactics have to be more than a few weeks of games (not rightly before the game), as you can see in the indicator
When my mid leauge team plays Man U they act as if they are star struck and just stand there, they always miss or hit the post, or run around like checkens with their heads chopped off LOL. But they stop the sillyness when we play another mid leauge team.
