I have a medium world where i am building my village on the first island where i never used the portal, so those pesky hollow knights won't bother me while building, And i kinda like the serene feel the island got.
But it's up to your personal preference, Choose an island with a enviroment you like, If the mobs are a problem for you, you can always get those island deeds, which are the exact same as the island with mobs on them, including trees and bushes but without any mobs at all, and they never spawn there, and even on a medium world they feel kinda big, and are entirely flat. I suggest buying the deeds of those and decide where you wanna build.
I also advise making a home world and grinding world, just buy the deeds on your grinding world where you unlock all the islands for easy access and do the building on your home world, but again, it's personal preference, since i hate messing up the world i wanna build in, and will eventually unlock all portals on my home world aswell so i can build villages and towns on all my islands.
