If it says they look at it dismissively, they either can't equip it or use it.
set them to melee/range before doing so they don't start using a bow and sheild
you most common items that cannot be equiped to allys is Hats
i have never seen this line "they look at it dismissively" how do you get them to even say taht?

just a human, i want them to pickup random gear off the ground that is better than what they wear
And is what they're going to pick up cursed, or is it unappraised?
perhaps unappraised, perhaps cursed, i don't know. but i've never seen them pick up like, anything.
Do they say "I have no use for this"? I attempted to have her pick up glasses and she said that. Some armor/equipment, followers just won't equip.
what option did you pick to get them to pick things up?
If you're not dominating end game monsters, bows are a good choice for followers for several reasons. They don't move as much when fighting (easier to keep them off traps/magic spamming walls), are less likely to go inting into really strong melee stuff, and the dex you confer from leadership will improve their damage.
A group of humans or other gear-capable followers standing in the starting area quivering knockback/fire/etc are no joke, and a lot easier to keep alive than melee dudes following you and charging in. Keeping followers alive in the gnomish mines isn't trivial (you might get away with hiding some if you rush a direction). After that, it's significantly easier.
