1 - If the save file has _ever_ had the "[X] Cheats" option turned on
2 - If the "[X] +1 Life" option is turned on
3 - If you have a mod enabled which modifies map files
You can check to see if your achievements on a save file are disabled by loading that game and pausing the game. The bottom right of the pause menu will tell you if achievements are disabled.
Another thing that can prevent you from unlocking monster race achievements, is if you are using the "[X] Aesthetic Skin Only" option. You need to be a "true" Insectoid, so that shopkeepers and humans hate you, etc.
The game also has to communicate with Steam (or Epic if you have the game there) to verify achievement completion. So if you are disconnected, or if there's something wrong with the servers (this occurs on Epic more frequently) their servers may not award you with the achievement properly. This also means that currently DRM-Free versions of the game also do not have a way to confirm achievement unlocks.
I hope this is helpful information.
