Im having a bit of trouble with this achievement. I understand the event itself is pretty rare but I just want to make sure about something before wasteing more of my time. From what Ive seen it seems that the event only happens if Ted goes insane and he has a full beard. So does Ted need to be insane for the event to appear or do I just have to get Ted past the 35 day mark with the axe?

He doesn't have to be insane. Just full beard after 32-33 days. I got this event 2 or 3 times from ~50 runs. So yes, it's just rare event. You can easily do it on baby difficulty.
