Most games have no problem (even ones that detect cheating) with something like Fast Clicker. I use it with ARK and DnL to mainly do healing, which requires that you click the mouse a few hundred times. Sure I can click the mouse that many times (although not quite as fast as the program can), but why should I have to? Program actions like this are just asking to give you RSS injuries.
Now I haven't read the EULA word for word for Wurm, but if it doesn't allow mouse clickers, it should. A mouse clicker is just repeating some action you initiate maybe 10 more times or possibly a few hundred times. It isn't accomplishing anything you couldn't do (assuming you don't already have RSS).
Everquest even allowed a good deal of macroing even from third party programs. A lot of two boxers use it and another program. I can't remember the name of either but to some extent they are allowed.
On WO, it's pretty simple, sometimes, a GM pops in front of you, say hello and ask you a simple question. If your character is doing something, and you don't reply, you are banned. Period.
Well, you can achieve some actions that have been started before you go afk, but if you are chaining more than 5actions, and still don't reply, then you're using something to automatize the game, and you get banned.
So, for your friend disabled, no issue using an auto clicker, as long as he's in front of the screen and can reply to a GM while using the auto clicker. But if you go afk,and still using it, banned.
If you play solo, you do what you want, but i would advice to simply go GM and give you all the skills you want, you'll save time and effort. If you are on another server, i would ask GMs first, on my side, I would say no to that, and ban people who are doing actions while afk.
I think the OP specifically did want a clicker that allowed you to go afk, so that would normally be considered cheating. I read some rules about this after I saw this thread. On Wurm Online, even a weight to hold down a key if used afk is considered cheating.

Some Wurm players seem to be of the masochistic type, where the harder a task is and the longer it takes, the more they enjoy it
I like the building and terraforming possibilities myself. I do like to play on a server where it is sped up a bit. I will use any shortcuts the game allows, short of cheating, hence, I'm interested in this subject.
It does seem really hard to pin down in this game exactly what is allowed or what is frowned upon. Part of the confusion comes from its origin of Wurm Online, where GMs would actually kick you from the game for "cheating". That game was I think PvP and also subscription based. Wurm unlimited is a very different animal, not paid, it is controlled by individuals, not a corporation. Settings that are adjustable, and rules set per each server, not one set of rules. Also it has a single player mode, where you can be the GM.
for example. you need to go somewhere for a minute.
you turn on your auto clicker and mine where a ore vein is.
you get back and you mined up to the limit of 100 ore
it will NOT work as a bot.....
this is exactly what a bot does... automates acrtions for the user
wether you are sitting on the toliet or in front of your screen is a mute point.. the mouse is being auto clicked by NOT you.
technically this is a bot.. crude and limited in function true.. but still a bot.
is it cheating?.... that would have to be determined by the owner of the server you are playing on... since you say you are playing by yourself... you would have to ask yourself that question ;o)
does it go against the ELUA ? i donno.. read it and find out ;o)
for me, any use of third party software/hardware to automate an action is considered cheating... IF it is disallowed by the owner of the server you play on
