Quite easily built and it looks like the best scene layout to build it from would be the Scripting layout.
Select the Scripting layout, Click the + beside the name to create a duplicate of it, the name will become Scripting.001, Change the name to UV Editing.
Now just pull screen together and open the required screens.
Change back to the Default layout and do a File Save Startup File to make sure the new layout is saved to your Blender Start Up.

Also Happy Birthday, Pte Jack!
Yes. Hence "...if you haven't done any other changes...".
LOL, yup, I wanted to re-emphasize that, because lot of ppl will quickly read over the
"...if you haven't done any other changes you want to keep"
part and see the
"bring everything back to the way it was when you first started Blender"
part thinking that it means,
"Oh, from the last time I started Blender before I deleted the screen",
not realizing it meant setting everything back to the "Basic Factory Setting" that were set when Blender was first installed on their system.
Also, if ppl do set their systems back to Factory, they still have to do the File - Save Start up File
in order to keep the change or Blender will start up using the old start up file with the screen missing the next time they use it after closing down the session.
And thanks for the wishes.
