Report a translation problem
I've had this occur a few times before, thankfully it is a rare occurrence, it seems to be when I've done a lot of work in an open Blender window without ever shutting it down.
When it does happen I close the window and restart Blender and it works as expected again, unfortunately that won't bring back any work lost due to using undo before realising redo has stopped working properly.
I make quick copies of objects and move them to another layer before using undo if I think I may want to keep what I have done as a quick backup just in case this does happen.
I'm using 2.79 at the moment(I'm far too busy to move onto 2.8 right now), hopefully this no longer occurs in 2.8???
This has always worked this way, it's actually a great way to go back to an earlier point quickly when you have taken the model in the wrong direction. You just need to be aware of this, which you are, then use it to your advantage.
As for the reset to default I have not idea, sorry.
You can also load the last auto save which is usually 2 minutes or so old.
