To get the SMD bone shapes back again, just check Shapes back on.

If you don't want this bone shape to be in the model at all there are 2 ways to get rid of it.
1) Change the Bone Shape option when Importing the SMD/DMX/QC files.

If the shape already exists in the model and you want to get rid of it completely OR you've added bones and want them to display as smd bones, the process has to be done from Pose Mode.
2) Put the model into Pose Mode, select the bone you want to change. Select the Bone tab in the property panel and either delete the Custom Shape (which will change the display type to the current selected type in the Armature Property Panel)
or
change the Custom Shape from blank to smd_bone_vis (to change the shape from what it is now to the smd_bone_vis shape).
The downfall to this methos is that the shape has to be changed or deleted for EVERY bone you want to change ONE AT A TIME.
