What could cause a Mirror modifier for bones to break?
So basically my mirror modifier is not mirroring across correctly, I'm using it for rigging and I've aligned my cursor to centre, even set my origin to match my cursor too.

Anyway I'm creating my rig via bones on the left, middle and using the duplicated bones for the right side to mirror my bone structure but oddly my when I mirror the bones to the other side it disconnects my duplicated bones from my middle bone structure and oddly the bones move far to the left instead of to the right of my model. I'm also making sure it aligns by the Axis Local.

Not sure what's wrong, I have tested this on basic bones to make sure it wasn't something I did with my bones, I get the exact same problem. It's weird because I've done this before and been able to mirror bones fine so not sure what's causing the issue.

I'm even making sure to name them with the L./R., etc...

Edit - Half Solution was: It was because I had to set the 'pivot point to the 3d cursor'. So my bone structure was correct. Was a simple change in the options that fixed my problem.

The bones on each side have to be named the same with a L/R in the name. For example Bone.001.L and Bone.001.R, also the Mirror Option must be turned with the armature in Edit Mode under the Options tab in the Transform Menu. (Press T to turn on the left hand Transform menu in the 3d Viewer)

Here's a tip for you as well.

If you have the Mirror option turned on and you want to create Left/Right bones across the X axis from a center bone, you can use SHIFT+E to extrude the Left and Right bones. You can then Disconnect them from the parent and drag one of them, the mirror bone will drag to the proper position on the opposite side.

Well as far as I can tell I'm doing it right with no luck. Already tried turning on the mirror modifier but I keep getting the same problem. I understand the naming since I tend to duplicate 1 side then shift them to the opposite side then flip the names.

The problem I keep noticing is it seems to be ignoring the mirror effect partly (flipped correctly) but it ends up positioning it between my original bones on the left side instead of aligning to my right side, the mirrored bones disconnect from the middle bone structure.

It's weird because my origin seems to be aligned correctly and I can't seem to figure out why it's behaving this way. I know my bone structure is fine and I even tried a basic simple bone with a bone going left to replicate the problem and it keeps happening oddly.
