I'm not OP but I finished the story at 28 as well. It's not that uncommon. It's most likely OP stuck to the bigger side missions and drilled through the main missions without doing the shorter missions much. Which means all/most of the main missions were done around 1-2 levels lower than you're meant to be but it makes it harder. It also actually makes the last mission quite challenging (esp. if you go it alone) but pretty rewarding afterwards. Getting to 30 after that wasn't hard for me but I'm thinking OP just missed some things along the way because of their play style.
I really agree with Bat about safe houses being the key. Go back to ones you've already unlocked in higher zone areas and unlock ones you haven't found. You don't have to rerun missions at all (I didn't at least) but you might want to do a couple on hard for the extra boost if you can't find the extra xp.
That means you most likely kept up with most of the smaller side missions and collectables and went to more safe houses etc. OP finished the story, that doesn't mean "everything". He killed the last boss. I myself still have a ways to go upgrading base and I completed the main story at the same rate as him (Lv. 28). It makes the game a lot harder than it has to be and you die a lot but it isn't impossible and it is fun. :)
