Seen a lot of comments about talking, giving gifts, etc., but the bare bones requirement is to visit your spouse and then leave. If your spouse is capable of reproducing at the time, it should trigger eventually, but sometimes takes a few (dozen) tries. If you're playing the game normally, it is quite easy to overlook, but if you actively visit your spouse, it shouldn't take too long.
The one catch is, the women do have a recovery period after giving birth, and any historical/custom children will prevent the mother from giving birth during that period as well. I am playing Cao Cao in the early 190s and Bian Shi isn't giving it up for him because she's already popping out all his other kiddos.
You can only have 1 husband, but the wife can have more than 2 children if she marry again- as a widow or seizing gifted item as a ruler to break marriage bonds. Your children will join the husband's force regardless if you are free officer or from an opposing force.
