Mods, where are they at?
Saw someone play Exanima on stream with a harder beginning in the campaign, more zombies with different weapons, different places and I think I even saw him fighting ghouls before the portal too. I checked out the Nexus but there's naught there.

Where can I find mods for the game?

It's called Hell Mode, and you get it on the official Discord. Doesn't look like the thing for me.

Mods are made to be broken. Look at Skyrim, breaking mods 10 years after release...
