As for healing items on the first level, the farthest I've gotten was to the other half of the first level, some of them just starts chasing me down and others join in when I stand my ground so I've been able to clear the first half of the dungeon without getting hit.
If only there was ONE guaranteed Healing Salve in the first level.... Its not like there is one or anything.
Nope lets not tell him this, instead troll. +1
To the OP, theres one near the skull sign. About half-way through level one.
THERE IS?! I thought only 2-3 Healing Salves existed! and you could only get the 3 through RNG! Please share any footage or images if you have any, I would be happy to be proven wrong.

There are 6 health salves in the game currently. Half have permanent locations, the other half spawn randomly. There is also another method to heal located on level 6, this one is more RNG based but can net you 4+ full heals.
Huh, I didn't consider that part of the first level, so I didn't mention it.
One time I opened it, I got a chain shirt. Another time, I got a heavy layered cloth gambeson.
Also, if the AI begins to chase you, drop things on it to mess up their balance. This'll buy you some time to run. Realistically, you should be fine to engage in combat once you equip at least a tunic/coat, waistcoat (fixed location), gloves (fixed location), and a leather cap (on an AI). Like Derrin says, a good shirt can turn a blade...but avoiding combat is always a better option until you have solid layers.
Kind of a reducto-ad-absurdium of what a lot of folks here say.
This game is not your average run-n-loot game, man. Its slower paced combat and exp given for merely observing the world around you is in place for a reason. You have to take things slowly and with an open mind.
So do you wanna know where they are???
Read this: GIT GUD!

Sadly enough, Arena is my most-played mode in the game. I'm mostly talking about the unforgiving aspect of severe Perma-damage for the slightest of errors. And like others have said, even if you run from an "overly agressive AI", they just give chace (even if you drop stuff in their path, if you somhow can react that fast) until everyone you pass starts to fight you, forcing you to fight 4+ enemies with just a torch and barrel lid in the dark. That has nothing to do with skill, its just unfair and annoying.
