To get it working well on a modern PC, you can download the BOSLoader from this site - http://adoxa.altervista.org/bos/index.html which fixes almost all problems you would have getting it running.

It says in the Trivia:
"Devil's Island Pinball
A sixth table, Devil's Island, was made but not included with Balls of Steel. It was finally released in early 2000 as a seperate product - not an add-on. Also, it was distributed by Expert Software, rather than Apogee or GT Interactive."
While this is a stand alone product, it can be technically used as a balls of steel add on, especially because enhanced versions of BOS loader can actually play wildfires other pinball games, it can also be manually added to BOS and launched via bos
You can think of this table as an early example of disc locked content to an extent, it wasn't added, but can run/operate fine on the game
It was never meant to work within Balls of Steel, as it was not released or published by Pinball Wizards aka 3D Realms aka Apogee Software, and they don't own the rights to it. It was released as a separate standalone product by Wildfire Studios.
Even though they call it an "add-on" table and earlier on the 3D Realms news feed they refer to it as an "expansion pack" it actually was never released that way. It was always and only a standalone product.
The reason the BOSLoader (coded by some third party fans) works on all Wildfire Studios pinball games is because the games all use the same engine.
If you can get it to work, I'd be interested to know how you did it. It would require some fancy under-the-hood tweaking or renaming of the files, and even then you wouldn't have Devil's Island listed within BOS in the table selection screen. :)
