I was just wondering what the "New Game +" feature does in the Karamari Hospital dlc?

It adds a jumpscare, Spooky wears nurses outfit, and you can find a sword, which you can use for a genocide run, which changes the last note and demon child's dialogue. Also, monster in bathroom 4 a new scare, and you can get new death if you kill red chair shadow with sword than sit down in chair.

To get the whole experience, you must play the game twice. It only takes about 20 minutes the second time around when you use the sword.

But in terms of story, there is only one small change, and that is the demon child room at the end, after the cow. So whether or not it's worth seeing this change is up to you.
