Survivor Orcs come back the same?

So in all the previews it's been made a really big deal about how there's this genius physical system that means Orcs can come back all burnt up, or poisoned green, or they can have metal limbs put on when they get chopped up.

However, in nearly 50 hours of gameplay so far I've had a ton of "Survivors", some coming back even 5-6 times, and I've not seen one burned up Orc, one poisoned Orc, or even one single Orc with anything beyond a metal plate on their head.

In fact 90% of the time they come back looking exactly the same as they used to. I've had them comment that I threw them off a mountain or burned them to a crisp, but they look fresh as a daisy!

I've even had Orcs who were cut in two and lost both their arms (!) come back 3-4 times looking like they've just got back from a relaxing spa holiday, original arms and all.

Is that normal or is my game just bugging out? :\

No if you remove an arm they came back with a hook or some stiches
