Thank you! Actually it turned out I was missing Thaurband as well as Carach Angren.
Better still, I stumbled across a workaround that unlocked both of them and my Appendices are now all 100%.
SOLUTION: Go to the map and view at the country-wide level. Then hold + and let it auto-zoom into the nearest of the main cities. Zoom out again and look at the map. Click on map markers to open the ones missing from your Appendices (Durthang and Thaurband in your case). If you're lucky they'll now have a link to view Lore at the bottom of the text. Click that and hopefully it'll take you to a new Appendices entry. If not, move to a different area and hold + again to auto-zoom into another city. Lather rinse repeat.
