Some stories suggest that Orc females are usually kept aside in distant camps so that their child-rearing functions aren't impaired by the poor fellas' insane death tolls. Others suggest that Sauron's first "orcification" of the Black Numenoreans more or less operated like Fallout's FEV and obliterated secondary sexual characteristics on a species-wide level. You're technically fighting a bunch of female Orcs, in this case - it's just not readily apparent.
The third avenue, and the one explored by the Peter Jackson flicks, is that Orcs are vat-grown from an arcane process coined by Sauron. The Uruk would essentially be sapient sewage on two legs, constantly bursting out of more pods. The only exception would be the Uruk-Hai, or the Great Orcs, who supposedly came in a limited batch. Hence the scenes in "The Two Towers" that involve lesser Orcs and goblins digging out Uruk-Hai.
